Blizzard is overhauling Mythic items once again in Diablo 4, removing random number generation entirely from the creation process to give players direct control over their endgame gear. The update arrives as part of PTR 3.2.0, running from August 4 to August 11.

What Changed With Mythic Items
Season 14 introduced a compromise where Mythic Uniques dropped with two guaranteed stats and two random ones. Following continued player feedback, PTR 3.2.0 shifts the system to be far more deterministic by eliminating Mythic RNG completely.
The Horadric Cube now features a new "Upgrade Direct" recipe. Instead of outputting a random Mythic item of the same slot, the recipe converts an existing Unique item—such as a Tibault's Will with ideal affixes and masterworking—directly into a Mythic version while retaining all modifications. Players cannot use this recipe on Unmodifiable items.
Equipment Restrictions
Any Mythic created through the new Cube recipe receives a Crafted tag. Players are restricted to equipping only one Crafted Mythic at a time, though equipping multiple Mythics crafted at the Jeweler remains unrestricted.
